How to choose with confidence: what you can check before deciding.
You are thinking about having a treatment — or perhaps you are only exploring your options — and the choice in Barcelona is vast. The short answer is that there is no ranking that can settle this for you. A good aesthetic doctor — and the right clinic — can be recognised by three things you can assess yourself.
The doctor should be medically registered and the clinic officially authorised. The first consultation should focus on explaining, not selling. And the treatment plan should include not only what may be suitable, but also what is not advisable and why. In this guide, we explain how to check each point, step by step.
It is not the newest device or the Instagram account with the most followers. Treatments — hyaluronic acid, botulinum toxin and collagen stimulators — are tools available to many clinics. What changes from one pair of hands to another is what happens before any treatment begins: the diagnosis.
Two people with the same wrinkle may need different treatments because the cause may be different: in one person it may be the skin, while in another it may be a loss of deeper structural support. A good aesthetic doctor does not begin with “Which treatment do I want?” but with an earlier question: “What is causing what I see in the mirror?” The indication follows from that assessment. And from the right indication comes the result you are looking for: to look well, not different.
That is why the three things worth assessing are training and credentials (which you can verify) the first consultation (which you can observe) and the way the treatment plan is proposed (which you can evaluate). Let us look at them one by one.
This is the easiest part, yet one that very few people check. In Spain, aesthetic medicine must be practised by qualified medical doctors, and both the doctor’s credentials and the clinic’s authorisation can be checked in just a couple of minutes:
The doctor. Every doctor authorised to practise is listed in the public register of their professional medical association — in Barcelona, the Col·legi de Metges — as well as in the central register of the Spanish General Council of Medical Associations, which can be searched online by name or medical registration number. A reputable clinic will publish that number; if you cannot find it, ask for it. No doctor should be uncomfortable with a patient checking their registration: it is public information.
The clinic. Aesthetic medicine clinics require healthcare authorisation from the relevant authority. The registration number is usually displayed in the footer of the clinic’s website and on documentation visible at the premises.
This helps you rule out the greatest risk — being treated by someone who should not be treating you — but it does not yet tell you whether you have found the right doctor. That is what the next step is for.
The first consultation tells you a great deal. You do not need to be a doctor to assess it; you simply need to pay attention to the order in which things happen.
A thoughtful assessment begins by listening. What concerns you, how long it has bothered you, and what you hope to achieve. Next comes the examination: looking closely, assessing the tissues and understanding what is causing what you see. Only then should a reasoned proposal follow: what is appropriate for your case, what is not, and why. If the doctor recommends a different approach from the one you had in mind, they should explain the reasoning calmly rather than impose it. You should leave with a written plan and a quotation, then make your decision afterwards, at home, without anyone waiting for you to sign.
There are two specific details worth noticing. First, informed consent should be read and signed before any treatment, with enough time to ask questions — never afterwards, once the product has already been administered. Second, you have the right to know exactly what is being injected: the brand, the product and its identifying label. A doctor who works with reputable products should have no problem showing you this information; quite the opposite.
And one more point, which may be surprising: the best sign at a first consultation is not always a “yes”. When a doctor explains that a particular treatment is not right for you — because your anatomy does not call for it, because the timing is not appropriate, or because the expected result would not justify the procedure — you are seeing their clinical judgement in action. That well-reasoned “no” is one of the hardest things to find and one of the most valuable things to hear.
Dr Manuela Gheorghiu, aesthetic doctor and medical director of Clínica Provença, puts it this way: many patients arrive at their first consultation having already chosen a treatment — “I’ve come for hyaluronic acid filler” — when in fact the treatment should be the final decision, not the first.
The most useful thing you can bring to an assessment is not the name of a technique or a photograph of someone else. It is what bothers you, described in your own words: “I look tired”, “this area has started to sag”. From there, the doctor’s role is to translate that perception into a diagnosis — identifying which tissue layer is responsible: bone, muscle, fat or skin — and then into an appropriate indication.
When the process follows that order, the treatment fits the face. When the order is reversed, the face is made to fit the treatment. And that is, more often than not, the difference between a result that enhances your appearance and one that looks obviously treated.

You do not need a long checklist. A few signs are enough to help you decide with confidence:
None of these signs means you need to walk out immediately, but each is a valid reason to seek a second opinion. Medical societies themselves encourage second opinions, and a good professional will not take offence.
They are useful, but only within limits. Reviews can tell you how a clinic treats its patients: punctuality, friendliness and whether questions are handled well. That is real information and worth reading. What a review cannot tell you is whether the treatment indication was the right one for that particular face, because the person writing it has no way of knowing. And social media photographs show the cases that have been selected for publication.
That is why these signals work better as an initial filter than as a final decision. They can help you choose whom to visit, but not whom to trust with your face. The real decision is made with the information you receive at your first consultation, because that is the only information based on your case rather than someone else’s.
Once you have a specific concern you want to understand — even if you are not sure which treatment you need, or precisely because you are not — an assessment is the natural next step. Its purpose is to translate what bothers you into a diagnosis and explain whether there is anything worth treating.
And if you are still at the stage of looking around, comparing clinics or simply getting used to the idea, there is no need to rush. This kind of decision benefits from time. Visit more than one clinic if you need to: comparing consultations will give you more useful information than any article, including this one. The right doctor will still be there when you are ready.
Ultimately, everything you have read here comes down to one idea: the best aesthetic doctor for you is not the one who promises the most, but the one who explains the best. You now know what to verify, what to look for at your first consultation and which questions to ask. With that information, the decision no longer has to be an act of faith.
