Thigh firmness is not only about weight or how much exercise you do. In many cases, it is not fat: it is the skin, which has lost support.
If the skin on your legs does not look firm, it does not mean you have done anything wrong. Firmness does not depend only on weight or exercise, but on the collagen that supports the skin — and that requires a different approach.
Here we explain what poly-L-lactic acid treatment does, when it makes sense, and when it does not.
We provide guidance based on medical expertise.
You may already have tried creams or devices without noticing much improvement. There is a reason for that: skin laxity is not on the surface, but in the layer of the skin where collagen provides support. Poly-L-lactic acid is a biocompatible and resorbable molecule that acts precisely there. It does not fill: it stimulates your skin to produce its own collagen again. That is why we say it does not add volume, but rebuilds support.
This is also why the change is progressive. Unlike a filler, which is visible on the same day, here the result develops over time, as the skin gains firmness. It is slower, yes, but also harder to perceive as something ‘done’.
It is important to be clear about what it does not do. Poly-L-lactic acid does not remove fat or slim the leg, and it does not treat cellulite as such — although in some cases it can visibly soften its appearance. It works on skin firmness, which is something different. Nor does it replace surgery when there is significant excess skin. Understanding this from the beginning is part of choosing the right treatment.
The treatment is performed in consultation. Poly-L-lactic acid is injected into the area — using collagen stimulators such as Lanluma or Sculptra, depending on the medical indication — and in the following days a massage protocol is recommended to distribute it evenly. It is normal to notice some tenderness or small bruises during the first hours or days; these are usually mild and resolve on their own. The amount of product and dilution are adapted to your area and your case.
This treatment is well suited to mild or moderate laxity in the thighs or legs, when the skin still has good quality: when what is lacking is firmness, not when there is a large amount of excess skin.
It can happen to people of any body type, because firmness is not a matter of weight. You may be slim, have an average build or train every day and still notice sagging thighs, because this is not corrected by losing weight. It is common after pregnancy, during menopause — when the skin loses collagen more quickly — or after a stabilised weight loss.
Not all areas of the leg behave in the same way, which is why we do not expect the same response from each of them:
When is it not the right option? When there is significant excess skin, usually after major weight loss. No matter how much collagen we stimulate, that excess skin will not retract enough. In that case, the option that makes sense is surgery: a thigh lift. If this applies to you, we will explain it during the assessment and guide you, even if it is not something resolved here.

The goal is firmer, better-quality skin, not a change in shape. Over time, the area feels smoother and more supported, but it still looks like your skin: that is exactly the point. We are not looking for a striking effect, but for the result to be perceived simply as an improvement.
It is useful to know how the process evolves, because the beginning can be misleading. During the first few weeks you will not notice much; in fact, around the second or third week the area returns to its initial appearance and many people think the treatment has not worked. This is part of the process. Firmness usually starts to become noticeable after six to eight weeks and continues to consolidate over the following months.
The result usually lasts around two years. This is not a one-day treatment, but a phased process — and that gradual pace is what protects natural-looking results.

Before treating, we assess the proportions of your leg, the quality of your skin and how each change fits within the overall balance. Treating an isolated area is easy; doing it with clinical judgement is what changes the result.
We anticipate the effect of each session before performing it and respect the time your skin needs to integrate collagen. We prefer to under-treat and adjust later rather than overcorrect: it is safer and more natural.
Your perception and expectations are part of the plan, not a formality. To that we add technical precision and continuous medical training, under the direction of Dr Manuela Gheorghiu.

They are different things, and distinguishing between them is what determines the treatment. Fat is volume; cellulite is related to the structure beneath the skin; and collagen loss is skin that has lost support. That is why you can have skin laxity even without excess fat: what is missing is firmness — collagen — and that is not a matter of weight. Poly-L-lactic acid acts on laxity by encouraging the body to produce new collagen and elastin, not on the other two. Identifying which of the three is really your case is the first thing we assess.
That is exactly the idea. Because the result develops gradually from your own collagen, it is difficult for it to look ‘done’: there is no overnight change, but skin that progressively recovers firmness. We plan conservatively: it is easier to add in a later phase than to correct an excess. The goal is not for the treatment to be noticeable, but for your skin to look better.
Around the third week, the area temporarily returns to its initial appearance while collagen begins to form. The visible change comes later, gradually, and continues to consolidate over the following months. This is the stage of the process that is most important to understand in advance, because it is when many people give up just as the treatment is starting to work.
Several sessions spaced approximately six weeks apart are usually needed, but the exact number depends on your area and degree of laxity, and is decided after seeing you. We do not give a fixed number in advance because that would mean deciding without assessing your case — and that is exactly what we avoid.
It is a resorbable molecule with a well-known safety profile and extensive clinical experience. The most common effects are mild and temporary: some tenderness or small bruises during the first few days. The effect that tends to cause most concern — small nodules under the skin — is uncommon and depends mainly on technique, dilution and the post-treatment massage; this is why who performs the treatment, and how, matters.
It depends on how much excess skin there is. When laxity is mild or moderate, this treatment makes sense. But when there is very severe excess skin, stimulating collagen is not enough: the appropriate route is surgical. If this is your case, we will confirm it during the assessment and guide you, even if the solution is not carried out here.
Because they act in different places. Creams remain on the surface and do not reach the layer where support has been lost; devices work in another way. Poly-L-lactic acid stimulates your collagen from within, in that specific layer. That said, it is not magic: it only makes sense if your concern is genuinely skin laxity. If what you have is something else, we will tell you.
