Not all arm skin laxity should be treated in the same way.

Before recommending any option, we assess the degree of laxity, skin quality and the starting point in each case. If poly-L-lactic acid is the best indication, we design a session-based plan tailored to your progression to achieve a gradual, natural-looking result.

And if it is not the most suitable option, we will guide you honestly towards the treatment that is most likely to give you the best result.

What poly-L-lactic acid is — and is not — for the arms

Poly-L-lactic acid is a collagen biostimulator. It is not a filler: it does not add artificial volume or change the shape of the arm overnight.

What it does is stimulate your own skin to produce structural collagen again, progressively restoring firmness, quality and texture from within.

This translates into a change that does not look “done”, but is perceived over time as firmer, smoother, more even-looking skin.

Improvement usually starts to become noticeable between weeks 4 and 8, consolidates between months 4 and 6 after the final session, and remains stable for around 18 to 24 months.

Why the arm requires more technical precision than other areas

Arm skin is thinner than the skin on the buttocks or thighs, and the subcutaneous tissue is less dense. This determines three technical decisions: high dilution, a deep injection plane and a retrograde fanning technique with a cannula. These decisions — not the molecule itself — are what make the difference in achieving an even result.

What it does — and does not do — in the arms

It improves firmness, texture and the crepey appearance of skin with mild to moderate laxity. It does not tighten truly redundant skin, shorten the arm or replace well-indicated surgery.

When treatment is the answer — and when surgery is

The key clinical question is not whether arm laxity bothers you. It is what kind of laxity you have.

To make this decision, we use a clinical classification that distinguishes different grades according to skin type and the degree of sagging.

Grade 1 — thin, crepey skin, without sagging

This is the scenario in which poly-L-lactic acid makes the most sense. The skin is not excessive, but it has lost quality.

Grade 2a — mild sagging with some fat

In these cases, treatment can be combined with liposuction to address both components.

Grade 2b — more evident laxity, with loss of support

Here, the role of the biostimulator may be combined with energy-based devices.

Grade 3 — clearly hanging skin, with significant fat

When the skin is already visibly hanging, no biostimulator can correct it. The indication is surgical, with assisted liposuction.

Grade 4 — excess skin after major weight loss

This is the typical situation after marked weight loss. The only effective solution is surgery.

Crepey skin is identified with a pinch test. It is thin, with a fine transverse wrinkled texture, and does not form a hanging fold when the arm is extended. This is the scenario in which poly-L-lactic acid offers the greatest value.

Redundant skin falls under gravity when the arm is held horizontally, creates a clear fold and often extends towards the axillary wall. In cases with significant excess skin, no biostimulator will contract that skin.

During the assessment, we perform a pinch test, observe the fold imprint with the arm extended and take photographs in standardised positions. You leave that appointment with a decision, not an estimate.

Is this your case?

It is best suited when:

Laxity is mild to moderate (stages 1 to 2b discussed above), the skin has a crepey appearance due to solar elastosis, chronological ageing or menopausal hormonal changes, and your weight has been stable for at least six months.

It is also a natural indication in women aged 40 to 55 who start to notice changes in their arms before other areas and want to preserve tissue quality without resorting to surgery.

It may also be appropriate, with more restrictive criteria, when:

Weight loss has been moderate, has been stable for at least six months, and the residual laxity is moderate.

During active weight loss or in more advanced laxity, we postpone treatment. Rushing it does not improve the outcome: it compromises it.

It is not the right option for you if:

The arm skin is already clearly redundant and hangs significantly when the arm is extended. If weight loss is still ongoing. If you have a personal history of keloids or hypertrophic scarring. If you are pregnant or breastfeeding. If you have active autoimmune or connective tissue disease.

There is no abrupt overnight change. The result is built over your skin’s own biological timeline.

Structural analysis before proposing treatment

We assess the arm using medical classification, pinch test and standardised photography. That is what guides the decision between poly-L-lactic acid, combination with an energy-based device or referral for surgery.

Criteria for not treating

If the degree of sagging corresponds to stage 3 or 4, we tell you in that same appointment and guide you towards trusted plastic surgery care.

Technique adapted to a demanding area

The arm is not treated in the same way as other areas.

It requires high dilution, a deep plane and homogeneous product distribution. That is why every treatment is performed medically, without delegation, adapting each decision to your case.

Follow-up and adjustment throughout the process

Between sessions, we assess progress with comparative photography and adjust the plan if needed.

The goal is not to complete a set number of sessions, but to achieve an improvement that is consistent with your starting point.

Frequently asked questions about arm skin tightening treatment

This is a common concern, and understandably so: arm skin is thinner, and any irregularity is noticed sooner than in other areas.

For precisely that reason, this treatment requires the technique to be adapted to the arm, rather than applying the same protocol used for the face or other areas.

We work with three key decisions:

  • a high product dilution to avoid visible concentrations,
  • a deep, even injection plane with a cannula,
  • and a post-treatment massage protocol that helps distribute it correctly.


When these decisions are tailored to your case, the aim is not for the arm to look “treated”, but for the skin to recover firmness and texture gradually and coherently.

What you notice is firmer arm skin with a better texture.

There is no added volume because poly-L-lactic acid does not fill: it stimulates your own collagen. The improvement develops gradually over months, not from one day to the next, which is why it usually goes unnoticed by others.

Patients themselves often describe it as “I see my arms as they used to be, not as someone else’s arms”.

The usual guideline is two to three sessions per arm, spaced 4 to 8 weeks apart.

The exact number is not fixed in advance. It is decided after assessing your degree of laxity, skin quality and, above all, the response we observe after the first session.

One session is rarely enough for a stable result; three are not always necessary. That is why we work with an open treatment plan.

During the first 48 to 72 hours there is temporary swelling, which resolves during the first week.

The real improvement usually starts to be perceived between weeks 4 and 8 in the form of better firmness and texture. The result is assessed clinically 4 to 6 months after the final session, not before.

Expected duration is around 18 to 24 months.

A history of keloids or hypertrophic scars is an explicit contraindication to poly-L-lactic acid.

In active autoimmune or connective tissue diseases (lupus, scleroderma, dermatomyositis, rheumatoid arthritis during flare-up), the response to a biostimulator is less predictable, so as a matter of clinical prudence we do not indicate treatment until the condition is stable and your specialist authorises it. Pregnancy and breastfeeding are also reasons to postpone.

We review all of this during the medical history. It is never a formality.

In consultation, we distinguish between crepey skin and clearly redundant skin using a pinch test and by observing the fold with the arm extended.

If you are stage 1 to 2b, you are a candidate. If the sagging corresponds to stage 3 or 4, the honest answer is referral for plastic surgery.

It depends on two variables.

The first is whether your weight has been stable for at least six months without significant fluctuations. Treating during active weight loss wastes the effect because the skin is still changing beneath the new collagen.

The second is the degree of laxity: in mild to moderate laxity, poly-L-lactic acid improves skin quality. If the skin is already clearly hanging (which is common after major weight loss), no biostimulator will contract it, and the correct answer is brachioplasty.

It is a conversation we have honestly.

In certain profiles, the combination provides real clinical value.

Microfocused ultrasound or radiofrequency with microneedling produce immediate contraction through thermal effect and prepare the tissue; poly-L-lactic acid maintains that gain over time through the deposition of new collagen. At least 2 to 4 weeks should separate both procedures so inflammatory responses do not overlap.

By contrast, combining it with other biostimulators in the same area and session is not supported by clinical consensus and does not improve the result.

Combination only makes sense when each technique contributes something the other cannot provide. If that is not the case, we do not recommend it.
