Bruxism is not always purely a dental issue. In many cases, there is an underlying component of muscular hyperactivity that can lead to jaw tension, masseter muscle fatigue, and associated discomfort.
When a night guard is not sufficient or muscular overload persists, a targeted medical assessment of muscle activity may be necessary to determine the most appropriate approach.
At Clínica Provença in Barcelona, each case is assessed from a functional and structural perspective, with particular attention to the balance of the lower third of the face. Medical decisions are carefully indicated and treatments are approached progressively, ensuring precision, safety, and respect for facial harmony.

The treatment involves the precise administration of neuromodulators into specific points of the masticatory muscles, most commonly the masseter, in order to temporarily modulate excessive muscle activity.
The goal is not to block normal function, but to reduce excessive contraction strength in carefully selected patients. Treatment is always indicated following an individualised medical assessment.
The effect is gradual and temporary, and its progression may vary depending on each person’s muscle characteristics and parafunctional habits.
Bruxism may involve repetitive or sustained contraction of the masticatory muscles, particularly during sleep, although it can also occur while awake.

For this reason, in some patients the approach should not be limited to dental management alone, but should also include a specific assessment of muscle activity.
A night guard can be useful to protect dental structures and help redistribute forces during sleep.
However, when muscular overload is the primary component, controlled modulation of the masseter muscle activity may be considered as an alternative or complementary approach under medical supervision. In selected cases, this can reduce the need for continuous night guard use.
It does not replace a dental evaluation when this is indicated. The decision is individualised following a medical examination and functional assessment.

Yes, “Botox” is a commonly used colloquial term. From a medical perspective, we speak of facial neuromodulation using prescription medicines that are always prescribed and administered by a qualified physician.
Suitability for treatment is always confirmed during consultation. Particular caution may be required in individuals with certain neuromuscular disorders, during pregnancy or breastfeeding, or when taking medications that interfere with neuromuscular transmission.
For this reason, a detailed medical history and prior clinical examination are an essential part of the decision-making process.
Neuromodulators are prescription-only medicines. Their administration must be carried out under medical supervision and following an individualised assessment.
As with any medical procedure, side effects may occur, although in most cases they are mild and temporary.
It is common to experience mild discomfort, slight redness, or a small bruise at the injection site. In some patients, there may also be a temporary sensation of reduced strength when chewing very hard foods. This adaptation is usually gradual and well tolerated.
After the procedure, we provide personalised aftercare instructions for the first hours and days, with the aim of supporting optimal recovery and minimising potential discomfort.
In addition, a follow-up appointment is scheduled if necessary to assess response and determine whether any adjustment is required.
Improvement usually appears progressively in the days following treatment and becomes established at around two weeks.
The average duration of effect typically ranges between 4 and 6 months. This may vary depending on the intensity of the bruxism, muscle activity, and each patient’s individual response.
Depending on clinical progression, it may be necessary to adjust the treatment plan or schedule maintenance sessions to preserve muscular balance and help prevent recurrence.
The aim of treatment is not to block muscle function, but to modulate its hyperactivity. For this reason, planning focuses on reducing excessive contraction strength while preserving normal chewing function.
In some patients, during the first days or weeks, there may be a slight sensation of reduced strength when chewing very hard or dense foods. This is usually a gradual adaptation and does not interfere with normal eating.
A conservative dosing strategy and phased approach allow for adjustments if needed, always prioritising the balance between relieving muscular overload and preserving normal masticatory function.
It is usually mild and short-lived discomfort.
It is an outpatient procedure. In most cases, you can resume your normal routine the same day, following simple aftercare recommendations.
It depends on the underlying cause of the bruxism. In cases where muscular overload is the predominant factor, it may reduce the need for continuous night guard use. The decision is always individualised.

Dr. Manuela Gheorghiu is a specialist in Aesthetic Medicine, founder and Medical Director of Clínica Provença, a medical aesthetic clinic in Barcelona. She combines her clinical practice with university teaching and advanced medical training, staying continuously up to date to offer evidence-based, safe and effective treatments.
She graduated in Medicine and Surgery with highest honours from the Carol Davila University of Medicine and Pharmacy in Bucharest. Her medical degree is officially recognised by the Spanish Ministry of Health, allowing her to practise as a licensed physician in Spain since 2018. Her specialist training in Aesthetic Medicine was completed in Spain, where she obtained her postgraduate qualification from the University of Córdoba.
She regularly participates in international congresses, courses and advanced training programmes, integrating the latest developments in aesthetic medicine with a rigorous clinical approach and a global vision of aesthetic rejuvenation.
