Supporting the scalp environment where hair grows, while the follicle can still respond.
There is a stage, before hair loss becomes obvious, when the hair simply starts to lose strength: it grows finer, has less body, and the scalp feels less healthy. If you have started looking for solutions, you have probably come across the term “polynucleotides” more than once, often accompanied by bold promises and very little detail.
Polynucleotides are a non-surgical medical treatment designed to improve the environment around the hair follicle — including microcirculation, inflammation and tissue quality — so that existing hair can grow under better conditions.
They do not act on the hormone responsible for genetic hair loss, and that distinction is essential: they are highly suitable in some cases and only one part of the plan in others. Your assessment determines where they fit.
We provide guidance based on medical expertise.
Polynucleotides are highly purified DNA fragments of biological origin that have been used in aesthetic medicine for years because of their regenerative effect on tissues. When delivered into the scalp through microinjections, their purpose is not to “feed the hair” or force it to grow, but to improve the environment in which the hair develops.
They work in several complementary ways: helping to reduce local inflammation, supporting the microcirculation that supplies oxygen and nutrients to the follicle, and improving the quality of the scalp tissue. When the follicle is still active but functioning in a compromised environment, improving those conditions may help the hair remain stronger, gain body and reduce reactive shedding.
It is important to understand what they do not do, as this is where most confusion arises. Polynucleotides do not act on DHT, the hormone involved in androgenetic alopecia; therefore, in genetic hair loss, they do not replace a treatment that directly addresses that cause.
They also cannot create new hair where the follicle has already been lost: if an area has been without hair for years, a different approach is required. Nor are they a universal treatment that works in the same way for everyone. Their role depends on what is causing your hair loss — something we prioritise at Clínica Provença before recommending any procedure.
Polynucleotides perform best when the objective is to support and preserve rather than restore an area where follicles have already been lost. They may be suitable if you have recently noticed a reduction in hair density or quality, if your scalp is prone to irritation or inflammation, or if you prefer to begin with a gentle regenerative approach before considering other options. They can also be a valuable complementary treatment: alongside therapy that addresses a hormonal cause, or after a hair transplant to support recovery of the treated area.
They are not the best standalone option for advanced androgenetic alopecia — where treatment aimed at controlling the hormonal cause plays the central role — or for areas where the follicle is no longer present, which may require hair transplantation. In those situations, we will explain this as clearly as we would recommend the treatment when it is appropriate.
Because the starting point varies considerably from one person to another, the first step is always to understand the type of hair loss you have.

The realistic aim of polynucleotide treatment is to improve the conditions in which your follicles function, so that the benefit can be reflected in the hair they produce. In practice, this may mean better control of reactive shedding, hair with somewhat greater body and thickness, and a healthier scalp environment.
Changes are progressive and follow the natural timing of the hair growth cycle, which cannot be accelerated. As a general guide, improvements in hair quality and reduced shedding may begin to be noticed during the first few weeks, while more visible changes in density are usually assessed around the third or fourth month, as new growth cycles are completed.
One point we prefer to explain from the outset is that the benefits of polynucleotides last for a variable period depending on the individual. For this reason, a maintenance plan may be appropriate in many cases to help sustain the improvement. Understanding this from the beginning allows you to decide with realistic expectations.

We listen to what concerns you and what you hope to achieve: when you first noticed the change, how it has progressed, whether there was a possible trigger, what you have previously tried and what you would like to improve.
We assess the condition of your scalp and hair follicles: the pattern of hair loss, hair quality in each area, whether inflammation or scalp sensitivity is present, and what may explain the changes — a hormonal cause, reactive shedding, a nutritional deficiency or a combination of factors. If necessary, we may request blood tests.
We provide a reasoned medical recommendation: whether polynucleotides are appropriate for your case, either alone or as part of a broader plan, or whether another approach is likely to offer greater benefit. Sometimes the answer is polynucleotides; sometimes another technique; and sometimes a combination.
You leave with a plan and a quote, and you decide afterwards, calmly.

Scientific evidence on polynucleotides is promising but still emerging, and it is currently better established for skin than for the scalp. What we do observe in clinical practice is good tolerability and improvements in hair quality and the environment surrounding the follicle.
Rather than asking whether they “work” in the abstract, the more useful question is whether they are appropriate for your specific case. During the assessment, we determine whether your situation is likely to benefit meaningfully. If it is, we will recommend them; if another treatment is likely to be more effective, we will explain that instead.
This is the most common source of confusion. Androgenetic alopecia is driven by a hormone, DHT, which progressively weakens the follicle. Polynucleotides do not act on this hormone: they support the follicular environment, but they do not address the underlying cause. For this reason, they cannot stop genetic hair loss on their own.
That does not exclude them from the treatment plan: they may be a useful complement to therapy that directly addresses the hormonal cause, helping to improve scalp conditions while the other treatment performs its role. What we would not do is recommend them as the only treatment for genetic alopecia, because that would not meet the needs of your case.
All three approaches share a common principle — stimulating the follicle rather than adding volume — and none of them acts on the hormone responsible for genetic hair loss. The difference lies in the source of the regenerative stimulus and the specific role each treatment may play.
PRP uses growth factors from your own blood: it requires a small blood draw, and its composition depends on your biological condition at that time. Exosomes provide a concentrated and standardised package of regenerative signals. Polynucleotides, rather than primarily sending signals, help condition the environment in which the hair grows — supporting microcirculation, inflammation control and tissue quality — which can make them particularly suitable for reactive scalps or when the aim is to support and preserve hair quality.
Even so, the distinction between polynucleotides and exosomes is not absolute, as their effects overlap considerably. Choosing between them is a diagnostic decision rather than a fixed rule.
The treatment is delivered through very superficial microinjections into the scalp. Each session involves a series of small injections rather than a single injection. Most patients tolerate the procedure well.
Afterwards, mild redness, local sensitivity or a small bruise may occur and usually resolves within one or two days. There is no significant downtime: you can return to your normal routine while following the simple aftercare instructions we provide.
Polynucleotides are obtained from marine-origin DNA and undergo a highly controlled purification process that removes proteins, which are the components most likely to cause a reaction. For this reason, the risk of allergy is considered low, and the product used is a CE-marked medical device.
Your medical history nevertheless remains essential. If you have a known fish allergy or any other relevant history, we review it carefully during the assessment before recommending treatment.
Polynucleotides are also not considered a first-line option during pregnancy or breastfeeding, in people with coagulation disorders, or when there is an active scalp condition. We review these situations carefully before starting treatment.
As a general guide, polynucleotide hair treatment is usually planned as a course of 3 to 5 sessions, spaced approximately two weeks apart, followed by maintenance treatment when indicated.
Improvements in hair quality and reduced shedding may begin to be noticed during the first few weeks, while more visible changes in density are usually assessed around the third or fourth month.
