Vertical lines above the upper lip can be treated medically without surgery. In selected cases, a hyaluronic acid filler specifically designed for this area can soften them without changing the shape of your lips.
You may first have noticed it when your lipstick began to bleed into fine vertical lines above the lip. The skin here is particularly thin, and repeated movement —speaking, smiling and making facial expressions— tends to leave its mark earlier than in other areas.
The good news is that this is also an area where a carefully indicated treatment, using the right product at the correct depth, can achieve a meaningful improvement with very little product.

Vertical lip lines, sometimes called smoker’s lines or lipstick lines, are the fine vertical wrinkles that develop above the upper lip. They result from two combined processes: over time, the skin in this area loses collagen and thickness, while the muscle around the mouth folds it thousands of times a day as we speak, eat and make facial expressions. Eventually, some lines remain visible even when the face is at rest. They can also appear in people who have never smoked, although smoking may make them more pronounced.
The treatment involves injecting a hyaluronic acid filler specifically selected for this delicate area: lightly cross-linked, fluid and designed to integrate into a superficial layer of the skin. Its purpose is not to add volume, but to improve hydration and support the skin from within, softening the wrinkle without altering the shape of the lip. This is the technical key to treating the perioral area: a filler intended for another part of the face, or placed at the wrong depth, may create unwanted fullness. The right product, placed in the correct plane, blends smoothly into the tissue.
What this treatment is not: it is not lip augmentation, nor is it intended to erase every sign of expression. Pronounced lines can be visibly softened, but dynamic lines will still appear when you move your mouth, because your lips remain natural and expressive.
The appointment takes around 30 minutes. We prepare the area with numbing cream, and the filler itself contains a local anaesthetic, so discomfort is usually mild. You can return to most normal activities afterwards. Mild swelling or small bruises may occur during the first few days and usually resolve on their own. An improvement is visible immediately, while the final result settles over one to two weeks as the initial swelling subsides and the product fully integrates.
Vertical lip lines filler is most suitable when the lines are already visible while your face is at rest. These are known as static wrinkles: lines that have become etched into the skin. This is where hyaluronic acid filler tends to be most effective, whether the lines are fine or more pronounced —the injection strategy changes, but the underlying treatment principle remains the same.

The aim is a smoother upper lip area, with softened lines and better-hydrated skin, without changing the shape of your mouth. In everyday life, the improvement is noticeable in practical ways: lipstick is less likely to bleed, the skin reflects light more evenly and the face appears more rested overall.
The result settles progressively and remains discreet. People who know you are likely to notice that you look better, without necessarily knowing why. Results typically last around 9 to 12 months, although this varies according to your skin, metabolism and lifestyle.
As the effect gradually fades, we assess whether maintenance is appropriate. In patients whose skin responds well, maintenance appointments often require less product than the initial treatment.


We begin by listening to what concerns you and what you hope to improve. We then assess the perioral area in detail: the depth and pattern of the lines —fine or pronounced, present at rest or only with movement—, the quality and thickness of the skin, the activity of the muscle around the mouth and the structural support of the lip. This allows us to identify what is causing the changes you see.
We then give you a medically reasoned recommendation: what is suitable for your case, what is not, and why. Sometimes the right answer is a different technique; sometimes, no treatment at all. You leave with a clear treatment plan and quotation, and decide afterwards, without pressure.

The answer lies in the technique. Vertical lip lines are treated with a lightly cross-linked hyaluronic acid that is much more fluid than the type used to add volume. It is placed in the superficial layers of the skin.
Its role is to support and hydrate the wrinkle from within, not to create fullness. The shape of your mouth, your profile and your expression when speaking or smiling are preserved. If our assessment shows that your lines require a different strategy to achieve precisely this natural result, we will explain it clearly before treatment.
Both situations exist, which is why a medical assessment should always come before treatment. Very superficial, diffuse lines may sometimes respond better to treatments that improve skin quality than to direct filler injection.
Very deep lines may require a combination of hyaluronic acid filler and other techniques, or the structural support of the area may need to be addressed first. During your consultation, we’ll assess your case and explain which treatment option makes sense for you.
For upper-lip lines that are already visible at rest, hyaluronic acid filler is the reference treatment: it supports the skin and softens the crease that has become etched into it.
Botulinum toxin acts on the muscle rather than filling the wrinkle, and its use in this area is very limited. The muscle around the mouth is involved in speaking, eating and drinking, and excessive relaxation can interfere with essential movements.
You will notice an improvement from the first day, but during the initial 48–72 hours some swelling may temporarily affect how the area looks and feels. This is normal, settles on its own and should not be considered the final result.
The final appearance settles over one to two weeks, once the filler has fully integrated. Results then typically last 9 to 12 months, with individual variation depending on skin quality, metabolism and habits such as smoking or sun exposure.
We use medical-grade, biocompatible and resorbable hyaluronic acid fillers. The body gradually integrates and metabolises the product. Common side effects are generally mild and temporary —such as redness, swelling or a small bruise— and usually resolve within a few days.
As a precaution, treatment is postponed during pregnancy and breastfeeding. At our clinic, we do not use permanent fillers, as they are associated with long-term complications that can be avoided by choosing a resorbable material.
Yes. In many cases, this is the most coherent approach because vertical lip lines rarely develop in isolation. Depending on the medical assessment, treatment may be combined with procedures that improve skin quality around the mouth, with nasolabial fold treatment when nearby folds affect the overall balance, or as part of facial harmonisation when it is more appropriate to assess the face as a whole rather than focus on one individual wrinkle.
