You may notice it more in photos than in the mirror: the cheekbone is less defined than before and the centre of the face looks slightly flatter, with a tired expression that does not match how you feel. In most cases, the issue is not the skin: it is the deep support of the midface, which gradually loses volume over time.
When the indication is right, hyaluronic acid allows us to restore that support with precision and bring projection back to the cheekbone, without surgery. Whether this is your case —and how much product is needed, often less than you might imagine— is decided during the medical assessment.

With age, the midface loses support: the cheekbone structure undergoes slight bone resorption and the deep fat that supports it decreases. The result is a less defined cheekbone and a flatter-looking centre of the face, which also provides less support to the under-eye area and the beginning of the nasolabial fold.
The treatment involves injecting a hyaluronic acid gel —a molecule naturally present in the skin— into the deeper planes of the cheekbone area, close to the bone, to restore support exactly where it has been lost.
The key difference lies in the approach: the aim is not to add volume so the cheekbone stands out, but to restore the projection it once had, allowing the rest of the face to sit more harmoniously on it again. That is why, before injecting, we study how each support point will affect the whole face: cheek, under-eye area and nasolabial fold. A well-treated cheekbone does not draw attention; the face simply looks more rested.
It is also important to clarify what it is not. This is not a treatment to change the shape of your face or recreate someone else’s cheekbones: it works with your anatomy, not against it. And it does not replace other solutions when the main issue is different: if skin laxity or descent of the cheek is predominant, another technique —or a combination of treatments— may make more sense than adding product.
The session is straightforward. The product is injected according to the area and the treatment plan, and the appointment usually takes around thirty to forty-five minutes. You can return to normal activities the same day, with minimal precautions that we explain before you leave. Mild swelling or a small bruise for a few days is common. The result can be seen immediately and continues to settle over the following weeks as the product integrates into the tissue; we then review it with you in consultation.
This treatment is usually well indicated when what you describe sounds like this: the face looks flatter or more tired even when you are well rested; the cheekbone, which used to be more defined, has lost definition; the under-eye area or nasolabial fold is beginning to show without significant excess skin; or you have lost weight and your face has reflected it more than expected.
It can also help with mild asymmetries and in faces that, due to their anatomy, have never had much projection in this area and want to harmonise it discreetly.
There are situations in which it is not the first option. If laxity is predominant —skin that has sagged, rather than volume that is missing— adding product does not address the cause and may overload the face. In these cases, we usually assess facial skin tightening treatments first.
If you already have previous fillers that haven’t been assessed, the first step may be to understand what’s there and how they’ve behaved over time, rather than adding more. During pregnancy and breastfeeding, we postpone treatment as a precaution. That’s what the consultation is for: to determine what’s appropriate for your specific case.

The first thing that is restored is projection: the cheekbone becomes more defined again and the midface no longer looks flat. With this, something less obvious but more important improves too: the overall expression of the face, which appears more rested. By restoring support, the transition between the under-eye area and the cheek softens, and the nasolabial fold tends to become less noticeable because it receives the support it had lost, without necessarily needing to be filled directly.
The result evolves naturally: the body gradually reabsorbs the product, without sudden changes. With the high-density hyaluronic acid fillers used in the cheekbone area, the expected duration is around twelve to eighteen months, and in some cases up to two years. When the effect starts to soften, we assess with you whether maintenance makes sense.

We analyse the midface layer by layer —bone, deep and superficial fat, and skin— to understand what explains what you see: a cheekbone that has lost bony support is not the same as a cheek that has descended, and each situation requires a different response.

The technique is designed specifically to avoid that. The “overfilled” look some people associate with dermal fillers is not caused by hyaluronic acid itself, but by where it is placed and how much is used: injecting too superficially, using excessive amounts, or treating isolated areas without considering the overall facial proportions.
By working in deep planes, using conservative amounts and anticipating the effect of each support point on the overall face, the cheekbone regains definition without widening the face or altering your smile. And if, during the assessment, we see that your anatomy does not need it —some cheekbones already have enough support— we will tell you.
Discomfort is mild. Our products contain local anaesthetic, so the sensation is usually more pressure than pain. After the appointment, you can return to almost all normal activities: we will ask you to avoid intense exercise, extreme heat and pressing or massaging the area for a few days.
Some swelling or a small bruise may appear, which can be covered with make-up and usually disappears within a few days. If you have an important event coming up, tell us during the assessment so we can schedule the session with enough margin.
When performed by a doctor with specific training in facial anatomy and with medical-grade products, this is a treatment with a high safety profile and decades of use in aesthetic medicine.
The most common effects are mild and temporary: swelling, redness or bruising at the injection points. Relevant complications are uncommon and are mainly related to technique and knowledge of the injection planes; this is why who performs the treatment matters more than any other factor.
At Clínica Provença, we do not use permanent fillers, and we postpone the treatment during pregnancy and breastfeeding as a precaution, because there are not enough studies in these stages.
As a general guide, the result lasts between twelve and eighteen months, although this depends on the product used, the amount injected, your metabolism and your lifestyle. The high-density hyaluronic acid fillers used in the cheekbone area are among the longest-lasting and can last up to two years.
Reabsorption is gradual, not sudden: the effect softens little by little. When this happens, maintenance can be considered; depending on your treatment history and current situation, less product may be needed than in the first session.
It depends on the cause, and that is exactly the question we answer during the assessment. In many faces, the nasolabial fold becomes more pronounced because the midface has lost support and the cheek rests on it; in these cases, restoring support in the cheekbone softens it more naturally than filling it directly, because it acts on the cause rather than the consequence.
In other cases, the fold has its own structure and should be treated directly, or both areas may be combined with small amounts. The order is determined by your anatomy; that is why we study it before recommending anything.
Yes, and it is often the most sensible option when the diagnosis supports it. Cheek fillers can be part of a broader treatment plan: combined with facial skin tightening treatments when skin quality also needs support, with hyaluronic acid in the under-eye area when the transition with the cheek requires it, or alongside the treatment of other folds as part of global facial harmonisation.
When several areas are involved, we usually work in phases, respecting the integration time of each step: your perception of the result between sessions is part of the planning.
