You raise your eyebrows hundreds of times a day without noticing: when you speak, listen or feel surprised. Each time, the forehead folds into horizontal lines that gradually begin to remain visible over the years.
Treating the forehead well requires particular care: the frontalis is the only muscle that raises the eyebrows. Relaxing it too much, or in the wrong areas, can make the eyes look heavy.
That is why Dr Manuela Gheorghiu does not plan forehead treatment in isolation. She first assesses how much your eyebrows rely on this muscle, and uses that information to determine the injection points and doses for your individual case.

Horizontal forehead lines are expression lines created by the frontalis muscle each time you raise your eyebrows. At first, they are only visible during movement; over time, repeated folding leaves an imprint on the skin and the lines also become visible at rest.
The treatment involves placing microdoses of a neuromodulator at specific points in the frontalis muscle to reduce — rather than eliminate — the strength of its contraction. The skin folds less, the lines soften and the forehead looks smoother and more rested. This is not a filler or a skin-tightening treatment: it does not add volume or alter facial structure; it simply modulates the muscle force that folds the skin.
This is where clinical judgement makes the difference. Because the frontalis is responsible for lifting the eyebrows, treatment must achieve a careful balance: enough relaxation to soften the lines, while preserving sufficient support so the eyes do not look heavy or tired. In some people, the forehead also works harder to compensate for eyelids or eyebrows that naturally tend to sit lower. Identifying this before treatment is essential to prevent an undesirable result.
The session takes only a few minutes and requires no downtime. You can continue with your day, although you should avoid rubbing the area or doing intense exercise during the first few hours. The effect develops gradually over the following days and settles at around two weeks, when we review the result.

In some cases, the forehead should not be treated on its own, or it should be treated with particularly conservative doses: especially when the eyebrows depend heavily on the frontalis muscle to remain in position.

We begin by listening to what concerns you and what you expect from the treatment.
We assess your forehead both at rest and in movement: the strength of the frontalis muscle, the position of your eyebrows and eyelids, and how much they rely on this muscle for support. This explains the lines you see — and determines where treatment can be performed safely and where a more cautious approach is needed.
Based on this assessment, we give you a considered clinical recommendation: what is appropriate for your case, what we would not recommend, and why. Sometimes the answer is a different technique; sometimes, no treatment at all.

A feeling of heaviness or a lowering of the eyebrows can occur when the frontalis muscle is relaxed beyond what that person’s anatomy can tolerate — because the dose is too high, the injection points are too low, or the dependence of the eyebrows on this muscle was not assessed beforehand.
Prevention starts with the assessment. If your eyebrows need support from the frontalis, we use more conservative doses, place the injections higher, or reconsider the treatment approach. We would rather start cautiously and adjust at the review than overtreat and have to wait for the effect to wear off.
Yes. A ‘frozen’ look is not an inevitable consequence of treatment; it depends on how the dose is planned. Our aim is to reduce the intensity of the skin folding while preserving movement, so you can still express surprise, attention or doubt, simply with less marking of the skin. If anything can be refined at the two-week review, we adjust it.
Because the forehead does not work in isolation. The frontalis muscle raises the eyebrows, while the glabellar muscles pull them down. If only the lifting muscle is relaxed and the downward-pulling muscles remain active, the eyes may appear more hooded or tired.
For this reason, the appropriate indication in some cases is to treat the forehead and frown lines together — or even to begin with the frown lines. This is not about adding treatment areas; it is about achieving a balanced result. During your consultation, we explain what your individual case requires and why.
They will soften and, importantly, the repeated folding that makes them deeper will be reduced. However, when a line has been forming for years, the skin retains a degree of memory and a fine trace may remain even when the muscle is relaxed. If this applies to you, we will explain it during your consultation.
This is one of the most established treatments in aesthetic medicine, with a well-studied safety profile. The usual side effects are mild and temporary, such as a small bruise, redness or a feeling of tightness during the first few days, which resolves on its own.
The treatment is not performed during pregnancy or breastfeeding, in the presence of an active infection in the area, in certain neuromuscular disorders, or in patients with an allergy to any of its components. We review this with you during the consultation, together with the aftercare instructions.
