Whether you are considering treatment or have already had it, the question is much the same: what should you take care of, and for how long?
Aftercare is simple and involves only a few precautions: gentle cold compresses if there is swelling, no heat or intense exercise for a couple of days, and letting the lips settle without manipulating them. What is most reassuring, however, is knowing what is normal to experience in the first few days, what is best avoided and when it is worth calling the clinic. Here is everything you need to know before and after treatment.
There are three things that genuinely help, and all three are easy.
First: tell your doctor about any medication and supplements you take. Some — such as aspirin, ibuprofen or certain vitamin supplements — can make bruising more likely. This does not mean stopping anything on your own: it means making sure your doctor knows, so they can assess it with you and, if appropriate, adjust the timing of the treatment.
Second: if you have ever had a cold sore, tell the doctor before your appointment. Injections can reactivate the herpes virus in people who are prone to outbreaks, and knowing this in advance allows preventive measures to be considered.
Third: come with clean skin and no make-up on the area, and avoid alcohol for a couple of days beforehand. Both measures can help reduce irritation and bruising. We apply numbing cream before starting to make the treatment more comfortable.
The first thing to know is that the lips you see when you leave the clinic are not the final result. They are your lips plus swelling.
Swelling usually peaks within the first 24 to 48 hours — it is common for the lips to look fuller than expected or slightly uneven for a short time — and begins to reduce noticeably from the third or fourth day. By around two weeks, the hyaluronic acid has integrated into the tissue and the lips show their settled shape. That is the right time to assess the result.
It is also common to have some localised bruising, tenderness to the touch and a degree of dryness that can be surprising: the lips may feel tight or peel slightly during the first few days. A gentle, fragrance-free lip balm is enough to keep them comfortable while they recover.
All of this is part of the tissue’s normal response to treatment. It is temporary, expected and generally compatible with everyday life: most patients return to their usual routine the same day.
Aftercare can be summed up in one idea: for a few days, make things easy for your lips so the product can settle where it has been placed.
Heat for the first 24–48 hours. Avoid very hot drinks, saunas, steam rooms and direct sun exposure. Heat dilates blood vessels and may increase swelling. If you are going to be outdoors, a lip balm with sun protection is a good idea.
Intense exercise for 48 hours. Increased blood flow can encourage swelling and bruising. Walking and gentle activity are fine.
Pressure on the lips. Avoid touching, rubbing or pressing them during the first few days, as well as intense kissing for the first 48–72 hours. If you normally sleep on your stomach, try to spend the first few nights on your back with your head slightly elevated: as well as protecting the area, this can help the swelling settle sooner.
Make-up on the area for 24–48 hours. The needle entry points need time to close. You can apply make-up to the rest of your face as usual.
Alcohol for the first 24 hours. As before treatment, it can increase swelling and bruising.
Exfoliants and facial treatments on the area for the first week. If you have a dental appointment, it is also best to leave a two-week gap: extensive manipulation of the mouth while the filler is settling is best avoided.
For swelling, cold is your ally: apply cold compresses for short periods, always with a cloth between the cold source and your skin. Never apply ice directly.
There are two points when patients commonly start to wonder whether everything is normal, and in both cases the internet often gives a worse answer than the reality.
The first is day two: you look in the mirror and see more lip volume than you expected. This is the peak of the swelling, not the result. Judging lip filler during the first 48 hours is like judging a cake halfway through baking. The true shape becomes apparent at around two weeks, which is why at Clínica Provença we schedule a review: so we can assess the outcome with you once it has actually settled.
The second is small lumps. Feeling a few firmer areas during the first two weeks is common — filler that is still integrating, together with some local swelling — and they usually resolve on their own. What we do not recommend is massaging them yourself: incorrect massage can move the product and compromise a result that was carefully placed. The vast majority of recoveries involve nothing more than a few days of swelling. Even so, it is important to know the difference between what is normal and what is not, because in the latter situation timing matters.
Call the clinic if you develop severe pain that is getting worse rather than better, areas of skin that become pale or change colour noticeably, swelling that continues to increase after the third day, or any signs of infection — marked local heat, discharge or fever. These situations are very uncommon, but they can be treated, and treatment is more effective when started promptly.

If you have not yet had treatment and are still gathering information, these recommendations are useful for more than just knowing how to look after your lips afterwards: they also give you an idea of the standard of care a reputable clinic should provide before and after the injections.
If you have already had lip filler and something still does not feel right after the two-week settling period, that does warrant a medical assessment. Shape, symmetry and how the lips feel can all be examined and, when necessary, corrected.
And if your lips are fine and you were simply curious, you do not need to do anything. Many concerns during the first few days resolve in exactly this way: by giving the lips time to settle while knowing what to expect.
