There is no single non-surgical facelift. There is a plan decided before anything touches your face.

At Clínica Provença in Barcelona, we approach the non-surgical facelift as an aesthetic goal, not as a single treatment.

That goal is achieved by medically combining three clinical tools (hyaluronic acid, collagen stimulators and neuromodulators), planned in stages according to each patient’s anatomy. A natural-looking result is not a promise: it is the outcome of the analysis we carry out before indicating any treatment.

Dr Manuela Gheorghiu leads the treatment planning. We start with a full structural analysis of the face to decide which tool, in which order and at what dose makes sense for your case. And to tell you clearly when the right indication is no longer non-surgical.

Non-Surgical Facelift.

What is a non-surgical facelift?

“Non-surgical facelift” does not name a treatment: it names an aesthetic goal. And the first clinical decision — the one that weighs most heavily on the outcome — is not which technique we use, but which anatomical problem we are dealing with.

Ageing in the mid and lower face is driven by bone resorption, descent of the deep fat compartments and displacement of the retaining ligaments. What patients perceive as “sagging” is often a loss of internal structure that leaves the skin without support. Rebuilding that support is what creates the lifting effect. This is not about filling wrinkles: it is about restoring the foundation that has been lost.

From that structural diagnosis, three different clinical tools come into play. Each one addresses a specific anatomical problem; choosing between them — and combining them in the right order — is where the difference lies between a coherent result and an obvious one.

Hyaluronic acid

Indicated when the issue is loss of support: reduced cheek projection, a less defined jawline or descent of the midface. It is placed in a deep plane to rebuild the foundation the tissue no longer has.

Collagen stimulator

Indicated when the issue is tissue quality: laxity, thin skin or reduced dermal density. It stimulates the natural production of new collagen over 2–4 months after each session.

Neuromodulators

Indicated when the issue is not volume, but the way the muscles of the upper face pull: expression lines, asymmetries or descent in certain areas.

Is this the right treatment for you?

The question most patients bring to their first assessment is a binary one: “Will this work for me or do I already need surgery?” We answer it from clinical judgement, not commercial interest.

It is best suited when:

A non-surgical facelift has clear limits. It is important to understand them before starting.

This approach does not remove excess skin, nor does it replace surgery when an immediate change is needed or laxity is already advanced.

However, surgery does not in itself improve tissue quality: skin density, texture and regenerative capacity continue to follow their own natural ageing process.

That is why, in many cases, a non-surgical facelift is used to improve skin quality and help maintain the result over time.

These are not opposing approaches. They act on different planes and can complement each other.

How your plan is decided

Planning does not follow a fixed protocol and it does not start from what the patient asks for. It starts with facial analysis.

We assess what is really happening at a structural level: whether support is lacking, whether there is descent, whether skin quality predominates or whether muscular action is the main issue.

From there, we decide what to treat first and what to expect.

In some cases, starting with neuromodulators before restoring support accentuates the sensation of heaviness or descent. In others, reinforcing structure without taking tissue quality into account creates results that do not integrate well.

That is why order matters.

Not because there is a standard sequence, but because each intervention conditions the next.

The plan we propose is the result of that analysis, not of a predefined combination of treatments.

Global structural analysis as the starting point

Before discussing tools, we map the face. Facial proportions, bone support, deep and superficial fat compartments, skin quality and muscle tone. The indication for each tool is derived from that map, not the other way around.

Anticipating the impact of each tool on the whole face

Every decision is made with the cumulative effect in mind. What happens to the lower face if we reinforce the midface. Which areas are better left untreated today so as not to compromise planning six months from now. Anticipation avoids corrections later.

Planning in stages, with respect for biological timing

The plan is divided into stages because tissue needs time to integrate between interventions. We do not accelerate the timeline to “see results sooner”: that is exactly the mechanism that leads to over-treatment. If a case needs two stages, it has two. If it needs four, it has four.

Active communication between stages

Your perception of the result is part of the plan. After each stage, we review how the previous one has integrated, which expectations are being met and which ones require the next step to be recalibrated.

Technical precision applied to specific decisions

Precision is not an abstract concept. It is reflected in concrete decisions in every treatment.

Which layer of the facial tissue to work in according to your tissue quality, so the result integrates well and does not show.

Which technique to use in which areas in order to minimise risk and improve control over the outcome.

How the face is analysed and treated individually, because not all cases respond in the same way.

The difference is not in the product, but in how, where and why it is used in each face.

Frequently asked questions about the non-surgical facelift

The naturalness of the result depends on decisions made before treatment. The plan starts with an analysis of your anatomy (proportions, bone support, tissue quality) and is designed to reinforce what you already have, not to impose a new shape. Doses are calculated at the minimum effective level for each area. Stages are spaced so the change is gradual and is perceived as rest or improvement, not as intervention. The result we seek is the one people who know you notice without being able to identify why.

Over-treatment happens when several interventions are compressed into a single session or repeated before the tissue has integrated the previous one. Our staged plan exists to prevent that: each phase has a defined clinical objective, a dominant tool and an individualised dose. Between phases, we allow the time each tool needs to settle. Cumulative dose is tracked in writing; if a phase is not necessary, it is removed from the plan.

The number depends on the starting degree of laxity, tissue quality, previous treatments and the goal agreed during assessment. After the consultation, you receive a written plan with the indicative timing of each stage and of the first maintenance session.

Visible changes appear in stages: structural support with hyaluronic acid can be appreciated from the same day and settles over 2–4 weeks; neuromodulators reach full effect after 10–14 days; the biostimulator works progressively from 30 days to 4 months. The consolidated result usually lasts 12–24 months with annual maintenance. Actual duration varies according to tissue quality, habits (smoking, sun exposure, weight fluctuations), hormonal status and adherence to the plan. We give ranges because each of these factors changes the timeline.

Yes, when the combination is planned in stages and integration intervals are respected. The three tools act on different planes and through different mechanisms, and they do not present pharmacological interaction with one another. Relevant contraindications are assessed during consultation: pregnancy and breastfeeding, active infection in the area, coagulation disorders or anticoagulation that cannot be adjusted, autoimmune disease in an active phase, a history of granulomas from filler material, myasthenia gravis or myasthenic syndromes. Your full medical history is reviewed before indicating any tool.

The decision is not made by you alone, nor by us without you. It is made during the assessment, with your face in front of us and the structural map completed. There are specific clinical indicators that mark the boundary: significant excess skin with loss of retraction capacity, marked platysmal bands, advanced submental accumulation or established ligamentous descent.

When those indicators are present, the non-surgical approach may offer partial improvement, but perhaps not the result you expect. In those cases, we tell you openly during consultation: we would rather you invest your time and money in the right treatment, even if it is not ours.

The first step is to map the residual product. Where it is, how much remains, and how it is integrating or redistributing with movement and tissue ageing. Only with that map can planning be responsible: there are areas where simply adding more leads to visible accumulation, and others where the problem is that the previous product was poorly distributed and adding more makes no sense.

Assessment in these cases is longer and, quite often, the first recommendation is to wait, adjust with enzyme if appropriate, or reorganise the plan before injecting again. The goal is never to “cover up” a previous result with more product.

They are tools based on different mechanisms: they work through dermal energy, not injectables. In some cases, they complement our approach; in others, they do not.

Before choosing a device, the right question is what your face needs, and to what degree: structural support, tissue quality, muscle modulation or dermal energy. That decision is made in consultation based on your anatomy, not on a list of available technologies.

A non-surgical facelift is not a single treatment, but a plan that combines different tools (hyaluronic acid, neuromodulators and biostimulation) according to what your face needs.

That is why it does not have a fixed price. The cost depends on:

  • which structures need support
  • whether skin quality needs to be treated
  • the number of stages in the plan
  • and whether previous treatments condition the planning


In some cases, the plan may be more straightforward and in others more comprehensive.
