“Are you angry?” Perhaps you have been asked that before for no reason. The area between the eyebrows contracts hundreds of times a day —when you concentrate, look at a screen or react to bright sunlight— and this repeated movement can eventually leave a vertical line that conveys tension even when you feel relaxed.

Treatment with neuromodulators relaxes the muscles that cause frowning, using just the right amount to soften the line and give the eye area a more relaxed appearance.

Dr Manuela Gheorghiu selects the injection points and doses after assessing how your frown muscles work, because it is this clinical decision —not simply the product— that determines whether the result suits you.

What does frown lines treatment involve?

Frown lines —the vertical lines between the eyebrows— develop through the repeated contraction of a group of small but powerful muscles that pull the eyebrows inwards and downwards whenever you frown. At first, the line is only visible during expression; over time, it may also begin to show at rest.

The treatment involves placing small doses of neuromodulator at specific points within these muscles. By reducing the strength of their contraction, the skin no longer folds with the same intensity and the line softens. This is not a filler: it does not add volume or alter your facial features. It acts only on the muscle responsible for folding the skin.

There is an important consideration in the frown area: these muscles lie very close to those that support the eyebrow and eyelid. For this reason, the depth, exact position and dose of each microinjection are determined individually, with careful respect for the anatomical safety margins of the area.

The session is brief —just a few minutes in the clinic— and you can resume your usual activities straight away, following a few simple recommendations during the first hours. The effect develops gradually: it begins to become noticeable after a few days and settles at around two weeks, when we review the balance of the result.

When the line has been present for years and remains deep even at rest, a neuromodulator can still help —by preventing repeated movement from marking it further— but it may not remove it completely.

Treatment is not performed during pregnancy or breastfeeding, in the presence of an active infection in the area, or in certain neuromuscular conditions. These circumstances are reviewed during your consultation.

What is the assessment like?

We begin by listening to what concerns you and what you expect from the treatment.

We assess your frowning pattern: which muscles are involved, how strongly they contract and how they relate to the position of your eyebrows and eyelids. This explains the line you see —and determines where treatment is and is not appropriate.

Based on this assessment, we give you a considered clinical recommendation: what is appropriate for your case, what we would not recommend, and why. Sometimes the answer is a different technique; sometimes, no treatment at all.

Frequently asked questions about frown lines treatment

This is the best-known complication in this area, and we understand why it may be a concern. Eyelid ptosis —a temporary drooping of the eyelid— is uncommon and is directly related to where, how deeply and at what dose the product is injected. The frown muscles are close to those that lift the eyelid, which is why treatment must respect the anatomical safety margins of the area.

During the consultation, we also assess the natural position of your eyebrows and eyelids, as some anatomies require greater caution than others. Should it occur, it is temporary and resolves on its own; our aim is to prevent it from occurring in the first place.

Yes. That is precisely how the treatment is designed. The aim is not to immobilise the frown area, but to reduce the strength of the expression that creates the line. You will still be able to concentrate, show surprise or displeasure and move your eyebrows; the central fold will simply become less pronounced. The minimum effective dose is the amount that achieves this balance, which is why it is determined after observing your facial movements.

It depends on how long the line has been present. If it is relatively recent, relaxing the muscle will often allow the skin to smooth almost completely. If it has been there for years, the fold will have left a more permanent mark in the skin. The neuromodulator will soften it —and help prevent it from becoming deeper— but a fine line may remain.

In these cases, we will explain this during the consultation. It is better for you to know what to expect before starting treatment.

Neuromodulators have been used in medicine for decades, and their behaviour in this area has been extensively studied. The usual side effects are mild and temporary, such as a small bruise at the injection site, redness or a feeling of tightness during the first few days.

Treatment is not performed during pregnancy or breastfeeding, in the presence of an active infection in the area, in certain neuromuscular disorders, or if you are allergic to any of the ingredients. We review all of this with you during the consultation, together with the recommendations to follow after the session.

Yes. “Botox” is the colloquial name by which most people know this type of treatment. In medicine, we refer to neuromodulation using prescription medicines that must always be indicated and dosed by a doctor. The name matters less than the clinical decision: which muscles to treat, to what extent, and which ones to leave unaffected.

It can be treated on its own and is, in fact, one of the areas most commonly treated in isolation. However, the frown area, forehead and eye contour work together: relaxing one area can slightly alter how the others behave.

For this reason, we assess the entire upper third of the face during the consultation, even when the final plan is to treat only the frown area. If a combined plan would be appropriate in your case, we will explain why —and if it would not, we will explain that too.
