Collagen biostimulators, hyaluronic acid, thread lifts. Each does a different job well — and choosing the right one starts with knowing what your face actually needs.
If you have been comparing treatments for facial sagging, you have probably noticed one thing: every clinic seems to present its own technique as the best. The clinical reality is simpler — and more useful.
Facial sagging can be treated without surgery in appropriately selected cases, but there is no single “best” treatment in the abstract. The best option is the one that targets what is actually driving your concern. Here, we explain what each treatment does, when it may be the right choice and when it may not. During your consultation, we determine which approach fits your face.
The one that targets the layer responsible for what you see. That is the short answer — and the principle that puts everything else in order.
The face loses firmness through several processes at the same time: the bone that provides structural support gradually resorbs, the fat compartments that shape the cheekbones and cheeks lose volume and descend, and the skin produces less collagen. In each person, one layer tends to matter more than another — which is why a treatment that works very well on one face may not be enough on another.
What we can do is give the question “what are the best treatments?” the answer it really deserves: go through each option and explain which type of case it is best suited to. That is very different from suggesting that every treatment works for everything.
These are injectable treatments that do not create immediate volume. Instead, they stimulate your own collagen production in the deeper layers of the skin. The most widely used include poly-L-lactic acid (Sculptra) and calcium hydroxylapatite (Radiesse). Hybrid treatments such as HArmonyCa combine calcium hydroxylapatite with hyaluronic acid, adding immediate support alongside progressive collagen stimulation.
It is important to clear up a common misconception here: we are not talking about filling the area where sagging is visible. We are talking about restoring support where it has been lost — close to the bone, at key points that help support the rest of the face. When correctly indicated, the effect is one of repositioning: facial proportions are restored without anything looking obviously “filled”.
They reposition descended tissue using absorbable threads while stimulating collagen along their path. It is worth clarifying how long the effects last, as this often raises questions: the thread itself is absorbed within a few months, while the collagen produced along its path remains afterwards. The effect is real, although more subtle than the initial lifting effect.
Polynucleotides are purified DNA fragments with regenerative properties. When injected, they act as a biological signal that stimulates the cells responsible for producing collagen and elastin, while also improving deep skin hydration. They do not add volume or change facial shape — their role is tissue quality: elasticity, density and radiance.
These treatments act on the visible skin layer, stimulating regeneration through microneedling, either on its own or combined with agents such as exosomes or the patient’s own platelet-rich plasma.
There is a pattern we see repeatedly in consultation that generic content rarely explains: most people self-diagnose based on where they can see the problem.
They look at the fold, the cheek that has started to descend or a less defined facial contour — and search for a treatment “for” that specific area. But in the face, the place where a change becomes visible and the place where it originates are rarely the same: what appears lower down around the jawline often reflects a loss of support higher up.
That is why, when someone tells us they tried a treatment and it did not work, the first step is not necessarily to change the technique. It is to reassess the diagnosis. Often, the treatment was appropriate for one layer — it simply was not the layer driving that particular face.

Well-designed treatment plans often use more than one tool, and that is a good thing: these techniques do not compete with one another; they divide the work between them. That does not mean doing everything at once. The first step is the foundation that best explains the case — structural support if support is missing, tissue quality if that is the main issue — and other treatments can then be added in stages if needed, allowing each one to settle before moving on to the next.
The order matters as much as the choice of treatment. And that order comes from assessing the individual face and understanding what needs to come first. In fact, this is one of the most valuable things you can take away from a well-conducted facial sagging consultation.
Treatment is not always necessary. If the change you notice is very early — perhaps the skin looks slightly less plump, or the face appears a little more tired at the end of the day — and it does not bother you, consistent skincare and healthy habits are a perfectly reasonable response. The option to treat will still be there later. Facial sagging usually progresses gradually, and taking the time to decide is part of making a good decision.
It makes sense to seek an assessment when the change is starting to catch your attention in the mirror — a less defined facial contour, or the first folds that were not there before — and you want to understand what is causing it before it progresses further.
It is also worth consulting if you have already tried a treatment and the result fell short. Often, it is not that “nothing works for you”, but that a layer was treated that was not the main driver of your concern — and a more accurate assessment can change the approach.
And if laxity is more advanced, a consultation is useful precisely because it can clarify what aesthetic medicine can realistically achieve in your case and what may already fall outside its scope. Knowing that before investing in treatment is also a good outcome.
