Each technique addresses a different cause. The best option for you depends on what is actually behind the changes you see.
If you have recently noticed that your facial contour looks less defined or your skin feels less firm than it used to, there is one useful thing to understand from the outset: facial sagging is not just a skin issue.
The face loses support across several layers — bone, fat, muscle and skin — which is precisely why it can now be treated without surgery by targeting the layer responsible in each case. Here we explain why it happens, which effective options are available and when it may be worth seeking a medical assessment.
For years, sagging was thought to be a problem of skin simply ‘giving way’. We now know that the skin is only the visible layer of a process that begins deeper within the face.
Over time, facial bone gradually resorbs, particularly around the jawline, chin and eye area. This bone provides the framework that supports everything above it. At the same time, the fat compartments that shape the cheekbones and cheeks lose volume and slowly descend. And as the skin produces less collagen and elastin, it becomes less able to adapt to these changes and maintain its own support.
The result is what you see in the mirror: volume that once sat higher in the face appears to have shifted downwards, towards the jawline and the fold that runs from the nose to the mouth. It is not that there is ‘too much’ skin; there is simply less underlying support.
Some factors can accelerate this process — unprotected sun exposure, smoking and significant weight changes — while others are simply part of your individual makeup, such as genetics and facial bone structure. This is why two people of the same age can age in very different ways. And why understanding which layer is responsible for what you see is the first step towards treating it properly.
It is the first question almost everyone asks. A good skincare routine — daily sun protection, hydration and collagen-stimulating ingredients such as retinoids — can help preserve skin quality and slow its deterioration. It is worth maintaining consistently.
What is not realistic is expecting a cream to replace the bone or fat the face has gradually lost: skincare acts at the surface, while facial sagging begins deeper down, beyond the reach of any cosmetic product.
The same applies to facial exercises: the available evidence is limited and, at best, the effect appears modest. They may be harmless in most cases, but they cannot replace lost structural support.
The good news is that this deeper layer — the one creams cannot reach — can now be treated with medical aesthetic techniques. That is what we will look at next.
Rather than memorising the names of different techniques, it is more useful to understand the logic behind them: if the face sags because it loses structural support and tissue quality, treatment aims to restore support where it has been lost and stimulate the tissue so it can regain firmness. The main options fall into three groups:
Collagen biostimulators. These are injectable treatments that do not create immediate volume. Instead, they stimulate the body’s natural collagen production in the deeper layers of the skin. Firmness improves gradually over several weeks, and the result is perceived as denser, better-supported skin rather than something that looks ‘added’. They are often a cornerstone of treatment when tissue quality is the main concern.
Structural hyaluronic acid. When the underlying issue is loss of structure — in the cheekbones, chin or jawline — the goal is not to fill the area where sagging is visible, but to restore support where it has been lost. When appropriately indicated, the effect is one of repositioning: facial proportions are restored without anything looking added.
Thread lifts. These reposition descended tissue while also stimulating collagen along their path. They are rarely used in isolation and are more often combined with the treatments above when an additional lifting vector is needed.
Which of these approaches is appropriate — or which combination, and in what order — depends on which layer is responsible for the changes you see. In many cases, treatment is planned progressively: the foundation is addressed first, allowing each stage time to integrate before moving on to the next.
The most common mistake in assessing facial ageing, explains Dr Manuela Gheorghiu, is to focus on where the sign appears rather than where support has been lost. Someone concerned about the fold from the nose to the mouth or the first signs of sagging along the jawline often asks for that area to be treated directly. But treating the visible sign without restoring the support that caused it tends to add weight precisely where the face is already descending. This is one of the most common reasons results can look excessive or unnatural.
A second common situation is patients who have tried a single technique — a thread lift or a small amount of filler — and feel that it ‘didn’t work’. In most cases, the technique itself is not the problem; it was simply applied to a layer that was not driving the concern. That is why, in the clinic, the question is never ‘which treatment do you want?’ but ‘what is causing what you see?’. Sometimes the answer is a phased treatment plan. Sometimes it is just one procedure. And when laxity is advanced enough that surgery is the appropriate option, we say so just as clearly.

Not always. If the change is still subtle, barely crosses your mind and you maintain good habits — daily sun protection, not smoking and a stable weight — waiting is a perfectly reasonable option. Facial sagging progresses slowly, and there is no disadvantage in simply observing it for a while.
It is worth seeking a consultation when the change has become noticeable to you in photos or in the mirror and you want to understand what is causing it in your case; when you have tried treatments without the response you expected and suspect the issue may lie in a different layer; or simply when you would prefer to understand what is happening in your face and what options you have, even if you decide not to treat it yet. Understanding the cause is valuable in itself.
And if the degree of laxity is significant, an assessment is still useful: it helps clarify what medical aesthetic treatments may realistically achieve in your case and what has moved into the realm of surgery.
